Mitral protez kapak disfonksiyonu ve pulmoner hipertansiyonu olan gebede acil sezaryen operasyonunda anestezik yaklaşım (Olgu sunumu)

Mitral protez kapak disfonksiyonu olan gebelerde sezaryen operasyonu, hem anne hem de bebek yaşamını tehdit edebilecek olaylara yol açabileceğinden bu tür hastalarda anestezik yaklaşım özel bir önem gerektirir. Bir olgu nedeniyle mitral protez kapak disfonksiyonu olan pulmoner hipertansif bir gebede sezaryen operasyonunda anestezik yaklaşımı tartışılmıştır. Otuzbeş yaşında, daha önce mitral kapak replasmanı uygulanmış, 35 haftalık gebe hasta ciddi solunum sıkıntısı ve ortopne ile hastanemize başvurdu. Yapılan ekokardiyografik incelemede, mitral kapakta trombüs olduğu, kapak açılımının kısıtlandığı görüldü. İlgili kliniklerin ortak kararıyla acil sezaryen operasyonuna karar verildi. Ortopneik olan hastaya anestezi indüksiyonundan önce elektrokardiyografi, radiyal arter kanülasyonu ve internal juguler ven kanülasyonu uygulandı. Swan-Ganz kateteri yerleştirildi. Hastada arteriyel basınç 120/83 mmHg, nabız 131 dk-1, SpO2 %97, santral venöz basınç 15 mmHg, pulmoner arter basıncı 85/55 mmHg olarak kaydedildi. Preoksijenizasyondan sonra anestezi indüksiyonu etomidat, ketamin, kas gevşemesi ise rokuronyum ile sağlandı. Bebeğin doğumundan sonra pulmoner arter basıncı 65/45 mmHg olarak kaydedildi ve operasyon boyunca hemodinami stabil olarak seyretti. Doğum gerçekleştikten sonra yenidoğanın Apgar skoru 9 olarak saptandı. Anneye postoperatif 8. gün elektif mitral kapak replasmanı yapıldı. Mitral kapak hastalığı ve pulmoner hipertansiyonlu gebelerde anestezik yaklaşım, ventrikül fonksiyonlarını korumak, genel anestezi ve endotrakeal entübasyonun olumsuz hemodinamik etkilerinden kaçınmak, yenidoğanda opioidlere bağlı solunum depresyonu oluşmasını önlemek olarak özetlenebilir.

Anesthetic management for cesarean section in a patient with mitral valve prosthesis (Case report)

In this case study, we discuss the anesthetic management for cesarean section in a pregnant patient with mitral valve prosthetic dysfunction and with pulmonary hypertension. Anesthesia in such a situation requires a special approach to avoid risk to the lives of both mother and baby. At 35 weeks' gestation, a 35 year old woman who had previously received mitral valve replacement was admitted to our hospital with serious dyspnea and orthopnea complaints. An echocardiogram showed thrombosis in the mechanical heart valves, and there was a restriction at the valve opening. It was decided to perform an emergency caesarean section on the patient. Before induction the patient was monitored by electrocardiography, radial artery and internal jugulary vein were cannulatated. A Swan-Ganz catheter was inserted. The patient's blood pressure was 120/83 mmHg, with a pulse rate of 131 beats min-1 and SpO2 of 97%. The jugulary venous pressure and the pulmonary artery pressures were 85/55 and 15 mmHg respectively. After preoxygenation, etomidate and ketamine were used for anesthetic induction, muscular relaxation was performed with rocuronium. Within minutes after delivery of the baby, pulmoner artery pressure had decreased to 65/45 mmHg and the patient's hemodynamic status was stable for the rest of the operation. The Apgar score of the neonate was 9. On the 8th postoperative day, an elective mitral valve replacement operation was performed on the mother. The anesthetic approach to pregnant women with mitral valve disease and with pulmonary hypertension can be summarized as maintaining the ventricular function, eliminating the undesired effects of general anesthesia and endotracheal intubation and preventing the depressant effects of opioids on the ventilation of the neonates.
