KOLONOSKOPİ YAPILAN HASTALARDA PROPOFOL-KETAMİN VE MİDAZOLAM-KETAMİNİN ETKİLERİNİN KARŞILAŞTIRILMASI

Amaç: Kolonoskopi, tanı, tarama ve tedavi amacıyla yaygın olarak uygulanan endoskopik bir işlemdir. Hastalar açısından rahatsızlık verici ve ağrılı olması sebebiyle sedasyon ile yapılması önerilir. Çalışmamızda, kolonoskopik inceleme sırasında sedasyon için kullan›lan propofol-ketamin ve midazolam-ketamin kombinasyonlarının etkinlik ve yan etkilerinin karşılaştırılması amaçlandı. Yöntem: Yerel Etik kurul onamı alındıktan sonra ASA I-II risk grubu 18-70 yaş arası toplam 100 hasta çalışmaya dahil edildi. Hastaların işlemden önce anksiyete düzeylerini ölçmek için Spielberg’in "durumluk sürekli anksiyete testi" uygulandı. Grup MK’ya (n=50) midazolam (0,05 mg kg-1)+ketamin (0,5 mg kg-1), Grup PK’ya (n=50) propofol (1 mg kg-1)+ ketamin (0,5 mg kg-1) intravenöz (i.v.) olarak uygulandı. Sedasyon derinliği "Observer Assessment of Alertness and Sedation Scale (OAA/S) " ile değerlendirildi. Hastaların derlenme süreleri, taburculuk süreleri, ek ilaç ihtiyaçları ve yan etkiler kaydedildi. Bulgular: Demografik veriler açısından gruplar arasında istatistiksel olarak anlamlı fark yoktu. THE COMPARISON OF THE EFFECTS OF PROPOFOL–KETAMINE AND MIDAZOLAM-KETAMINE IN PATIENTS UNDERGOING COLONOSCOPY

Objective: Colonoscopy is an endoscopic procedure for diagnosis, screening and treatment. It is uncomfortable and painful, so sedation is preferred. We aimed to compare the efficacy and adverse effects of midazolam plus ketamine and propofol plus ketamine during colonoscopy. Method: After the ethics committee approval, 100 patients ASA I-II, age 18-70 were enrolled. State Trait Inventory (STAI) was used to measure the anxiety levels before colonoscopy. The MK group (n=50) recieved 0,05 mg kg-1 midazolam and 0,5 mg kg-1 of ketamine, the PK group (n=50) recieved 1 mg kg-1 propofol and 0,5 mg kg-1 of ketamine intravenously. Depth of sedation was evaluated by "Observer Assessment of alertness and Sedation Scale". Recovery, discharge times, additional medication and side effects were recorded.
