KETAMİN-PROPOFOL VE REMİFENTANİL-PROPOFOL İNDÜKSİYONUNUN LARİNGEAL MASKE YERLEŞTİRME KOŞULLARI AÇISINDAN KARŞILAŞTIRILMASI: İDEAL İNDÜKSİYON AJAN KOMBİNASYONU HANGİSİ?

Amaç: Laringeal maske (LMA) yerleştirilmesinde çeşitli intravenöz indüksiyon ajanları kullanılmaktadır. Genel anestezialtında başarılı LMA yerleştirmek için yeterli anestezi derinliği ve yeterli ağız açıklığı sağlamak gereklidir. Bu çalışmada ketaminve remifentanilin laringeal maske yerleştirme koşulları açısından kıyaslanması amaçlandı. Yöntem: Hastalara rutin monitörizasyon ve BIS monitörizasyonu yapıldıktan sonra zarf usulü ile hastalar iki gruba ayrıldı.Bir gruba 1 µg kg-1remifentanil 30 sn'de, sonrasında 2 mg kg-1propofol ile indüksiyon yapıldı. Diğer gruba 1 mg kg-1ketamin 30sn'de, sonrasında 2 mg kg-1propofol ile indüksiyon yapılıp LMA yerleştirildi. LMA yerleştirilmesi sırasında hemodinamik veriler,BIS değerleri, buna ilaveten LMA yerleştirme skoru ve yerleştirme süresi kaydedildi.Bulgular: Propofole eklenen ketamin ve remifentanilin laringeal maske yerleştirme koşulları açısından benzer etki gösterdiğibulundu. Hemodinamik olarak bakıldığında ketamin ile daha stabil hemodinami sağlandı. Remifentanil ile daha derin anestezisağlanırken, ketamin ile aralarındaki fark anlamlı bulunmadı. Scheme sınıflamasına göre, remifentanil ketamine göre daha iyisonuç sağladı.Sonuç: Ketamin veya remifentanil propofol ile birlikte laringeal maske yerleştirmede tercih edilebilecek ajanlardır.

COMPARISON OF KETAMINE-PROPOFOL VS REMIFENTANYL-PROPOFOL INDUCTION ON LARYNGEAL MASK INSERTION CONDITIONS: WHICH IS THE IDEAL INDUCTION AGENT COMBINATION?

Objective: Many intravenous agents have been used for laryngeal mask insertion. To achieve successful laryngeal maskinsertion, efficient anesthetic depth and mouth opening is necessary. This study aims to compare ketamine and remifentanyl effectson laryngeal mask insertion conditions. Method: After routine and BIS monitorisation, patients were divided into two groups by envelope method. One group received1 mcg kg-1remifentanil for 30 s and 2 mg kg-1propofol, the other group received 1 mg kg-1ketamine for 30 s and 2 mg kg-1propofolfor induction, and then laryngeal mask was inserted. Hemodynamic values, BIS values, laryngeal mask insertion scores andinsertion time were recorded.Results: Adding ketamine or remifentanyl to propofol showed similar effects on laryngeal mask insertion conditions. Ketamineshowed more stable hemodynamics. While remifentanyl achieved deeper anesthesia, no statistical difference was detected comparedto ketamine. According to Scheme classification, remifentanyl provided better results. Conclusion: Ketamine or remifentanyl with propofol are alternative agents for laryngeal mask insertion.
