Deli bal zehirlenmesi; olgu sunumu
Rhododendron familyası bitkileriyle beslenen arıların ürettiği ve içerisinde grayanotoksin bulunduran bal, ülkemizde deli bal, tutar bal veya acı bal olarak bilinir. Bu balın yenilmesi sonrasında doza bağımlı olarak insanlarda hafif sindirim sistemi irritasyon bulgularından, yaşamı tehdit edebilen bradiaritmiler, hipotansiyon ve santral sinir sistemi etkilenimine kadar birçok semptoma sebep olabilir. Öncesine ait herhangi bir kalp hastalığı olmayan 40 yaşında erkek hasta baş dönmesi, fenalık hissi, bulantı ve senkop yakınmaları ile kliniğimize başvurdu ve hastanın başvurmadan 2 saat önce Karadeniz balı yediği öğrenildi. Hastanın yapılan fizik muayenesinde nabzı 49 atım dk' ve kan basıncı 60/40 mmHg olarak ölçüldü. Çekilen elektrokardiyografisinde sinüs bradikardisi saptandı ve 1 mg atropin intravenöz olarak uygulandıktan sonra düzeldi. Hasta 48 saat sonra normal vital bulguları ve ritim ile taburcu edildi. Deli bal zehirlenmesinin tanısını koyabilmek için detaylı öyküyü içeren bir klinik değerlendirme yapılması genellikle yeterlidir. Karadeniz balına bağlı zehirlenme vakaları nadir görülmekle beraber her zaman akılda tutulması gereken bir durumdur. Bu vakalar zamanında tedavi edilmezse istenmeyen sonuçlara yol açabilir. Hastanemize Karadeniz balı yedikten sonra tipik zehirlenme bulguları ile başvuran bir hastayı literatür eşliğinde sunmayı amaçladık.
Mad honey poisoning; a case report
The honey of the beesfed with rhododendron family plants that include grayanotoksin is known as mad honey, wad honey or bitter honey in our country. Eating this honey dose dependency can cause a wide spectrum of symptoms, from non specific gastrointestinal symptoms to life threatening bradyarrhythmias, hypotension and central nervous system effects. A 40-year-old man with no history of heart disease or drug use was admitted to our emergency department after an episode of syncope, dizziness, nausea and distress. We learned that the patient's breakfast had included nearly 30 mg of honey, brought from a city in the western Black Sea region of Turkey, two hours before his admission. Physical examination showed bradycardia (heart rate, 49 beats min'1) and hypotension (blood pressure, 60140 mmHg). Surface electrocardiography revealed complete sinus bradycardia. At his initial evaluation by nurses, they noticed severe bradycardia which resolved after 1 mg atropine sulfate injection. He yvas discharged at the end of the two day. A clinical evaluation that includes a detailed history is generally sufficient to establish the diagnosis of mad-honey intoxication. Although the Black Sea Honey .(Mad Honey) toxicity is very rare, this possibility should be kept in mind. In case it is not treated, it can cause undesired effects. In the present review, the historical progress, clinical presentation and treatment modalities of mad-honey poisoning were analyzed in the light of the literature.
