Baryum sülfat aspirasyonu sonrası gelişen ARDS
Aspirasyorı katı ya da sıvı materyalin havayoluna ve akciğerlere inhalasyonudur. Orofarinks ve özefagusun anatomik ve fonksiyonel bütünlüğünü bozan bazı durumlarda aspirasyon riski artar. Radyokontrast olarak, baryum sülfat mukozadan emilmediği için güvenli kabul edilir ve çok yaygın kullanılır. Yanlışlıkla aspirasyonu sık görülür ve genellikle sessiz seyididir. Bununla birlikte, baryum sülfat aspirasyonurıa bağlı olarak gelişen ciddi akciğer sorunları ve ölümler bildirilmiştir. Bu yazıda gastrointestinal sistemin radyolojik incelenmesi sırasında baryum sülfat aspire eden ve sonrasında ARDS ve septik şok gelişen hastanın sunulmasının yanı sıra aspirasyon için predispozan faktörler, ayırıcı tanı ve tedavi tartışılmaktadır.
ARDS after barium sulphate aspiration
Aspiration is a term used to describe inhalation of solid or liquid materials into the airways and lungs.The disturbance of the anatomical and functional integrity of the orofarinx and esophagus can predispose the patient to aspiration. As a radiocontrast media, barium sulphate is considered safe due to is nonabsorbable property, and is widely used for upper gastrointestinal examinations. Accidentally aspiration occurs frequently during these examinations and is likely to be silent. However, severe lung pathologies and death after barium sulphate aspiration have been reported. We present a case of ARDS following barium sulphate aspiration and discuss the predisposing factors-for aspiration, differential diagnosis and treatment of ARDS secondary to barrium sulphate aspiration.
