Artroskopik diz cerrahisinde tek taraflı spinal anestezide izobarik bupivakain ve izobarik levobupivakainin retrospektif karşılaştırılması

Amaç: Bu çalışmanın amacı diz artroskopisi için tek taraflı spinal anestezi uygulanan hastalarda eşit miktarda bupivakain ve levobupivakainin anestezik ve hemodinamik etkilerinin retrospektif olarak karşılaştırılmasıdır. Gereç ve Yöntem: Ocak 2007 ve Aralık 2010 tarihleri arasında tek taraflı spinal anestezi altında elektif diz artroskopisi yapılan, 18-60 yaş arası ve Amerikan Anestezistler Derneği (ASA) sınıflaması I-II olan 61 hasta çalışmaya dahil edildi. Olgular Grup I %0,5 izobarik bupivakain 7,5 mg ve Grup II %0,5 izobarik levobupivakain 7,5 mg yapılanlar olarak 2 gruba ayrıldı. Duyusal ve motor blok pinprick ve Bromage skoruna göre değerlendirildi. Duyusal blok başlama zamanı ve süresi, motor blok başlama zamanı ve süresi ile maksimum duyusal blok seviyeleri ve KAH, O AB değişimleri açısından karşılaştırıldı. Bulgular: Gruplar arasında yaş, cinsiyet, operasyon süresi ve ASA bakımından istatistiksel olarak anlamlı fark yoktu (p> 0,05). Duyusal blok başlama zamanı, duyusal blok süresi, motor blok başlama zamanı ve motor blok süresi bupivakain grubunda anlamlı olarak daha uzun bulundu (p< 0,05). Buna karşılık Bromage skoru ve maksimum duyusal blok seviyeleri iki grupta da benzerdi (p> 0,05). Maksimum duyusal blok düzeyi Grup I'de torakal sekiz (T8), Grup II'de ise torakal on (T 10)'a kadar çıktı ve bu farkın istatistiksel olarak anlamlı olmadığı görüldü (p> 0,05). Grup I ve Grup II ayrı ayrı değerlendirildiğinde grupların kendi içinde kalp atım hızı (KAH) ve ortalama arter basıncı (OAB) değerlerinin istatistiksel olarak anlamlı olduğu görüldü (p< 0,05). Sonuç: Artroskopik girişimlerde tek taraflı spinal anestezi ile düşük doz izobarik bupivakain ve levobupivakainin hemodinamik etkileri benzer bulunmasına rağmen motor ve duyusal blok sürelerinin levobupivakain grubunda anlamlı olarak daha kısa bulundu. Bu nedenle cerrahi planlanırken bu durumun göz önünde bulundurulması ve levobupivakainin günübirlik anestezide bupivakaine alternatif olarak kullanılabileceği düşünülebilir.

The retrospective comparison of isobaric bupivacaine and isobaric levobupivacaine in one-sided spinal anesthesia for arthroscopic surgery

Objective: The aim of this study was to compare the anesthetic and hemodynamic effects of bupivacaine and levobupivacaine on the patients who undergo one-sided spinal anesthesia for knee arthroscopy, retrospectively. Material and Methods: A total of 61 patients with ages between 18 and 60 who had elective lower extremity arthroscopy operation between January 2007 and December 2010 were included in the study. Their ASA category was /-//. The patients were randomly divided into two groups as follows; Group I; isobaric bupivacaine 7.5 mg. and Group II; isobaric levobupivacaine 7.5 mg. The sensorial and motor blocks were evaluated according to pinprick and Bromage scores. The starting time and duration of sensorial, motor and maximum sensorial block was recorded in both groups and compared statistically. Results: There was not a statistically significant difference between the groups in terms of age, gender, operation duration, and ASA score (p> 0.05). The starting times of sensorial and motor blocks and the duration of these blocks were longer in bupivacaine group (p< 0.05). The Bromage score and maximum sensorial block levels were similar in both groups (p> 0.05). In Group I, maximum sensorial block level was T8, in Group II, it was T10, (p>0.05). When group I and II were considered separately, heart rate and mean arterial pressure were statistically different in each group (p< 0.05). Conclusions: The hemodynamic effects of bupivacaine and levobupivacaine were found similar during arthroscopic surgery, but motor and sensorial block durations were found significantly shorter in levobupivacaine group. As a consequence, this should be taken into consideration in surgery planning and the usage of levobupivacaine in day surgery anesthesia can be suggested as an alternative to bupivacaine.
