ANTİKOAGÜLAN KULLANAN HASTALARDA REJYONEL ANESTEZİ

Ortopedik cerrahiler sıklıkla rejyonel anestezi teknikleri ile gerçekleştirilir. Rejyonel anestezinin bu hasta grubunda morbidite ve mortaliteyi azalttığı gösterilmiştir. Bu cerrahilerin uygulandığı hastalar yandaş hastalıkları nedeniyle veya tromboemboli proşaksisi amacıyla antikoagülan tedavi almaktadır. Rejyonel anestezi uygulanan hastalarda antikoagülan tedaviye bağlı spinal ve epidural hematom gibi ciddi hemorajik komplikasyonlar ve nörolojik sekeller ortaya çıkabilir. Bu gibi komplikasyonları önlemek amacıyla sürekli güncellenen kılavuzlar yayınlanmaktadır. Klinisyen komplikasyonları önlemek için durumu hasta bazında değerlendirmeli, antikoagülan ilaçlar hakkında bilgi sahibi olmalı, kılavuzları takip etmeli ve kanama riskini girişimin yararına göre kıyaslayarak karar vermelidir.

REGIONAL ANESTHESIA IN PATIENTS RECEIVING ANTICOAGULANT THERAPY

Orthopedic surgeries are frequently performed under regional anesthesia techniques. Regional anesthesia has been shown to reduce morbidity and mortality in this group of patients. Patients undergoing these surgeries are receiving anticoagulant therapy due to comorbidities or thromboembolism prophylaxis. Hemorrhagic complications and neurological sequelae such as spinal and epidural hematoma due to anticoagulant therapy may occur in patients under regional anesthesia. Updated guidelines are published to prevent such complications. The clinician should evaluate the situation on a patient-by-patient basis, be informed about anticoagulant drugs, follow guidelines, and compare the risk of bleeding to the beneŞt of the procedure to prevent complications.
