Ağır tetanoz olgusu (olgu sunumu)

Tetanoz güvenli ve etkin aşısı yaygın kullanılmasına karşın mortalitesi yüksek olan önemli bir sağlık problemidir. Bu olgu sunusunda acil serviste immunoproflaksi önerilerine uyulmadığı için ciddi tetanoz gelişen 56 yaşında bir erkek hasta sunuldu. Yaygın kasılmaları olan olguda belirgin solunum güçlüğü oluştuğu için entübe edilip, sedasyon ve kas gevşemesi sağlandı. Tetanozda mortaliteye en fazla neden olan otonomik disfonksiyon, esmolol infüzyonu ile başarılı bir şekilde tedavi edildi. Yoğun bakımda kaldığı süre boyunca kültür sonuçlarına göre antibiyotik tedavisi düzenlendi. Yoğun bakımdaki 36. gününde mekanik ventilatörden ayrıldı ve daha sonra şifa ile taburcu edildi. Bu olgu sunusunda tetanoz immunoproflaksisi ile otonom instabilitenin tedavi prensipleri tartışıldı.

Severe tetanus (A case report)

Despite the widespread availability of a safe and effective vaccine against tetanus, it is still a significant health hazard with high associated mortality. This is the case study of a 56 year-old male patient who developed severe tetanus because tetanus immunopropyhlaxis guidelines were not followed at the Emergency Room. Following the appearance of general spasms, he began to have marked respiratory difficulty and required intubation, sedation and relaxation. The major life-threatening effects involving autonomic instability were overcome successfully with esmolol infusions. Antibiotic treatment was carried out according to culture results throughout his stay in the intensive care unit. On day 36, he was weaned from ventilation and later discharged. In this case report, tetanus immunopropyhlaxis and autonomic instability management is discussed.
