Vajinismus ile başvuran bir dissosiyatif kimlik bozukluğu vakası ve psikoterapisi

19 yaşındaki bir dissosiyatif kimlik bozukluğu vakasında öykünün derinleştirilmesi ile ifade bulan uzun süreli vajinismus ele alınmıştır. Dissosiyatif kimlik bozukluğuna yönelik özgül tedaviye başlanması ile birlikte vajinismus ortadan kalkınıştır. Kısa süre içersinde meydana gelen gebelik antidepressif ilaç tedavisine olanak tanımamış ve devam eden dissosiyatif süreç çerçevesinde yeni sorunlar ortaya çıkmıştır. Uygun psikoterapötik müdahalelerle hastanın sağlıklı bir doğum ile sonlanan gebelik süreci ve sonraki günlerde her hangi bir psikiyatrik komplikasyonla karşılaşması önlenmiştir. Hastanın psikoterapisi devam etmektedir. Toplumda en az şizofreni kadar sık görülen dissosiyatif kimlik bozukluğunun dikkatle aranması ve özgül psikoterapi ile ele alınması önem taşımaktadır.

Psychotherapy in a case of dissociative identity disorder presenting with vaginismus

A 19 year old patient with dissociative identity disorder and major depression who has had vaginismus for three years was treated with specific expressive integrative psychotherapy for dissociative disorder. As the patient developed pregnancy following successful psychotherapeutic intervention, untidepressive pharmacotherapy could not administered. During the whole period of pregnancy, the patient experienced several problems derived from the dissociative psychopathology which could be managed successfully. As a psychiatric disorder as prevalent as schizophrenia in the community, dissociative identity disorder should be screened carefully among clinical populations in order to intervene with specific psychotherapy.
