Şizofreni hastalarına verilen ruhsal eğitim ve telepsikiyatrik izlemenin hasta iĢlevselliği ve ilaç uyumuna etkisi

Giriş: Bu çalışma, şizofreni hastalarına klinikte verilen ruhsal eğitimin ve taburculuk sonrası yapılan düzenli tele- psikiyatrik (telefonla) izlemenin hastaların sosyal işlevselliğine ve ilaç uyumuna olan etkisini belirlemek amacıyla randomize kontrollü deneysel çalışma olarak yapılmıştır. Yöntem: Bu çalışma 01 Temmuz 2010-31 Mayıs 2011 tarihleri arasında Kayseri Eğitim ve Araştırma Hastanesi Kayseri Ruh Sağlığı Merkezinde izlenen ve araştırma ölçütlerine uyan deney (s=32) ve kontrol grubunu (s=30) oluşturan 62 şizofreni hastası ile yapılmıştır. Hastalar için Hasta Tanıtıcı Özellikleri Anket Formları kullanılmıştır. Deney ve kontrol grubundaki hastalara eğitim öncesi, eğitim sonrası (taburculuğu sırasında) ve altı aylık telefon izlemesi sonrası Tıbbi Tedaviye Uyum Oranı ve Sosyal İşlevselliği Değerlendirme Ölçekleri uygulanmıştır. Deney grubundaki hastalara verilen ruhsal eğitimde Şizofreni Hastalarına Yönelik El Kitabı kullanılmıştır. Taburculuk sonrası hastalar ile altı ay süresince her hafta düzenli ve ortalama 15 dakika süren telefon görüşmesi yapılmıştır. Telefon izlemesinde standart yarı yapılandırılmış yedi sorudan oluşan bir görüşme formu kullanılmıştır. Bulgular: Deney grubundaki hastaların eğitim sonrası ve telefon izlemesi sonrası tedaviye uyum ve sosyal işlevsellik puan ortalamalarının arttığı, eğitim öncesi ile eğitim sonrası ve telefon izlemesi sonrası puan ortalamaları arasındaki farkın önemli olduğu belirlenmiştir. Eğitim sonrası ve tele- fon izlemesi sonrası deney ve kontrol grubundaki hastaların tedaviye uyum ve sosyal işlevsellik puan ortalamaları arasındaki fark önemli bulunmuştur. Buna ek olarak, tedaviye uyum ve sosyal işlevsellik puan ortalamalarının grup ve zaman etkileşimi de anlamlı bulunmuştur. Sonuç: Bu sonuçlara göre, ruhsal eğitim ve telefon izlemesi ile yapılan telepsikiyatrik izlemenin hastaların tedaviye uyum ve sosyal işlevsellik düzeylerini artırdığı saptanmıştır. Şizofreni hastalarının ruhsal eğitim ve telepsikiyatrik izleme ile desteklenmesinin hasta üzerinde olumlu etkisinin olduğu ve evde hasta izlemesinde kullanılması önerilebilir.

The effects of psychoeducation and telepsychiatric follow-up on social functioning and medication adherence in the patients with schizophrenia

Objective: This randomized-controlled experimental study was conducted to determine the effects of psychoedu- cation in the clinic and regular telepsychiatric follow-up (via telephone) after discharge on social functioning, drug adherence of patients with schziophrenia. Methods: This study was performed on totally 62 patients, assignedst to experiment (n=32) and control (n=30) groups, from 1st July 2010 to 31 May 2011, being followed in Mental Health Center of Kayseri Training and Research Hospital and meeting research criteria. Patients characteristics were recorded by a form. For the patients in experiment and control groups; before education, after education and after 6-months telephone follow-up Scales of Medication Adherence Rating, Social Functioning were applied. In the psychoeducation process, the patients in the experiment group were given Manual of Information for Patients with Schziophrenia prepared by the researcher. After discharge, patients were interviewed by telephone for 6 months, every week regularly in average for 15 minutes. A standard, semi-structured interview form consisting of 7 questions were used for telephone follow-up. Results: For the patients in the experiment group, after physchoeducation and telephone follow-up, the mean scores for adherence to treatment and social functioning scores were increased, the differences between before and after education and after telephone follow-up were significant. Again, the differences between the mean scores of adherence to treatment and social functioning were significant after education and after telephone follow-up for the patients in both experiment and control groups. In addition, the group and time interaction of the mean scores for adherence to treatment and social functioning were significant. Conclusion: According to these results, the psychoeducation and telepsychiatric follow-up via telephone induced increases in adherence to treatment and social functioning for the patients. Supporting the patients with schizophrenia via psychoeducation and telepsychiatric follow-up had positive impact on these patients and these techniques are recommended to be used in patient follow-up at home.
