Sınır kişilik bozukluğunda benlik saygısı, duygu düzenleme güçlüğü ve duygulanım ilişkisi

Amaç: Sınır kişilik bozukluğu (SKB), duygusal, bilişsel, davranışsal bozukluklarla giden, ergenlik veya erken erişkin- likte başlayan bir psikiyatrik bozukluktur. Linehan’ın biyososyal kuramına göre duygu düzenleme güçlüğü (DDG), SKB’nin çekirdek özelliğidir. SKB’de benlik saygısının (BS) sınır (borderline) belirti şiddeti ve depresyonla ilişkili olduğu bilinmektedir. Çalışmamızda BS’nin benlik saygısının SKB çekirdek belirtisi olan DDG ve negatif duygulanım ile ilişkisini araştırmayı amaçladık. Yöntem: Polikliniğimize başvuran DSM-IV’e göre SKB tanısı konmuş ve Minne- sota Çok Yönlü Kişilik Envanteri ile tanısı desteklenmiş 26 SKB hastası ve 26 sağlıklı kontrol alınmıştır. Katılımcılara Sosyodemografik Veri Formu, Bilişsel Duygu Düzenleme Ölçeği (BDDÖ), Duygu Düzenleme Güçlüğü Ölçeği (DDGÖ), Rosenberg Benlik Saygısı Ölçeği (RBSÖ), Pozitif ve Negatif Duygu Ölçeği (PNDÖ) Beck Depresyon Ölçe- ği (BDÖ), Beck Anksiyete Ölçeği (BAÖ) uygulanmıştır. Bulgular: Gruplar arasında yaş, cinsiyet, eğitim durumu ve medeni durum açısından fark bulunmamıştır. SKB katılımcıların DDGÖ toplam ve alt ölçek puanlarında, PNDÖ’nün negatif duygulanım alt ölçeği (PNDÖn) ve RBSÖ’de sağlıklı kontrollerden istatistiksel olarak anlamlı düzeyde yüksek puan aldıkları görülmüştür. Regresyon analizlerinde depresyonun (BDÖ) yordayıcı değişkenlerinin negatif duygulanım (PNDÖn), BS’nin (RBSÖ) yordayıcı değişkeninin depresyon (BDÖ), negatif duygulanım (PNDÖn) yordayıcı değişkeninin ise duygu düzenleme güçlüğü toplam puanı olduğu bulunmuştur. Sonuç: Çalışmamızda DDG’nin negatif duygulanım yordayıcısı olduğu sonucu ile doğuştan gelen duygusal duyarlılığa ek olarak duygu düzenleme güçlüğünün de olumsuz duygulanıma katkıda bulunduğu görülmüştür. DDG’nin olumsuz duygulanımı yordayıcı etkisinin uyum bozucu davranışların tetiklenmesi yoluyla olduğu, çalışmamızda RBSÖ ile ölçülen süreğen BS ile DDG arasında ilişki bulunamamasının BS’nin süreğen DDG’nin durumsal bir görüngü olması nedeniyle ortaya çıkmış olabileceği düşünülmüştür. DDG’nin benlik saygısına etkisinin olup olmadığının geniş örneklemli çalışma- larla değerlendirilmesi uygun olacaktır. (Anadolu Psikiyatri Derg 2019; 20(5):506-514)

Relationship between self-esteem, emotion dysregulation and affect in borderline personality disorder

Objective: Borderline personality disorder (BPD) is characterized with emotional, cognitive, behavioral disorders, and its manifestation becomes evident in adolescence or early adulthood. According to Linehan's biosocial theory, emotion dysregulation (ED) is a core feature of BPD. Self-esteem (SE) is known to be associated with borderline symptom severity and depression in BPD. In our study, we aimed to investigate the relationship between ED, SE and negative affect. Methods: Twenty-six patients with BPD and 26 healthy controls were included. Sociodemog- raphic data form, Cognitive Emotion Regulation Questionnaire (CERQ), Difficulties in Emotion Regulation Scale (DERS), Rosenberg Self-Esteem Scale (RSES), Positive and Negative Affect Scale (PANAS), Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI) were our evaluation tools. Results: No difference was found between the groups in terms of age, gender, education and marital status. The total and subscale scores of the DERS, the negative affect subscale of the PANAS (PANASn), the RSES were found to be significantly higher in BPD than healthy controls. Predictors of depression in the linear regression analysis were negative affect (PANASn), and predictors of SE (RSES) is depression. Emotion dysregulation (DERStotal) was the predictive vari-able of negative affect (PANASn). Conclusions: In our study, DDG was predictor of negative affect and it was observed that emotion dysregulation itself contributed to negative affect in addition to the innate emotional sensitivity. The predictive effect of ER on negative affect was thought to be triggered by dysregulating behavior. No relationship was found between ER and SE because of evaluating ER state and SE trait. It would be appropriate to evaluate whether DDG has an effect on self-esteem with large sample studies in the future. (Anatolian Journal of Psychiatry 2019; 20(5):506- 514)
