İzmit depreminde hospitalize edilen depremzede çocuk ve ergenlerin klinik özellikleri

Amaç: Bu çalışmada izmit depreminde enkaz altından çıkarılarak Marmara Üniversitesi Tıp Fakültesi Hastanesi 'nde hospitalize edilen depremzede çocuk ve ergenlerin yaşadıkları ağır travmanın oluşturduğu ruhsal tepkileri ve psikopatolojiyi değerlendirmektir. Yöntem: Hospitalize edilen 49 depremzede çocuk ve ergenden dosyalarına ulaşılabilen 21 olgunun yaş, cinsiyet, yakınmalar ve tanı açısından dosyaları retrospektif olarak değerlendirilip veriler toplanmıştır. Ruhsal belirtiler DSM-IV tanı kriterlerine göre değerlendirilmiş, veriler ki-kare, Mann-Whitney U testi ile hesaplanmıştır. Grup sayılarının istatistik hesaplamalar için yeterli olmadığı durumlarda sonuçlar % olarak verilmiş, 'eğilim' olarak değerlendirilmiştir. Sonuç: Depremzede çocukların büyük bir bölümünün akut stres bozukluğu veya buna benzer semptom kümesi gösterdiği yapılan ilk değerlen-dirme ve izlemde saptanmıştır. İstatiksiksel olarak anlamlı fark bulunmasa da, bazı ruhsal belirtilerin kızlarda daha çok görüldüğü saptanmıştır. Ağır fiziksel travmaya maruz kalan çocuklarda uykusuzluk (X2=3.82, df=l, p=0.03) ve ağrı (X2=5.96, df=l, p=0.06) daha yüksek oranda bulunmuştur. Enkaz altında kalma süresi arttıkça duygusal izolasyon (Mann-Whitney U=22.500, p=0.032) artmaktadır. Tartışma: Ağır fiziksel ve ruhsal travmalardan hemen sonra akut stres bozukluğu (ASB) her yaşta görülebilmektedir. Araştırma grubumuzdaki çocukların hepsinde bu belirtiler saptanmıştır. Şiddetli fiziksel ve ruhsal travma olan enkaz altında kalma süresi arttıkça depremi yeniden yaşama hissi, duygusal izolasyon artmakta, enkaz altında kalış süresinin psikopatoloji için başlı başına risk faktörü olabileceğini düşündürmektedir. Bazı ruhsal belirtilerin sayı olarak kızlarda daha fazla bulunması ile, travmadan çok sonraki yıllarda depresyon, anksiyete bozukluğu gibi psikopatolojilerin kızlarda daha sık oranda görülmesi arasındaki ilişki, izleme çalışmaları ile aydınlatılmalıdır.

The clinical features of children who are hospitalized after earthquake

Objective: The aim of this study> is to assess the effects of trauma on children who are severely injured and hospitalised after the earthquake. Method: The records of 21 children who are hospitalised in various clinics of Marmara Medical Faculty after the earthquake is reviewed retrospectively. Psychiatric symptoms are classified according to DSM-IV diagnostic criteria. The data is analysed with khi square and Mann-Whitney U tests statistically. of the number of the patients in a group is less than requiredfor the statistical analysis, results are seen in the girls more than the boys. Children who are severely injured had insomnia (khi square=3.82, p=0.03), and pain (khi square=5.96, p=0.06) more than the others. The severity of emotional isolation (Mann-Whitney U=22.500, p=0.032) increased with the time spent under the ruins. Conclusion: Incidence of acute stress disorder (ASD) is high according to DSMIV, after severe physical and psychological trauma. Ali of the children in our group had ASD symptoms. Emotional isolation increased with the time spent under the ruins. Severe emotional and physical trauma, such as staying under a collapsed house seemed to increased the risk of psychopathology. in our sample, some psychiatric symptoms such as insomnia, hopelessness, emotional isolation are seen in the girls than the boys. follow-up studies are required in order to understand the long term psychiatric effects of trauma on children and adults.
