Çok sayıda ruhsal bozukluğu taklit eden bir yapay bozukluk olgusu

Yapay bozukluk, böyle davranmayı gerektiren herhangi bir dış etken yokken, hasta rolünün benimsenmesiyle karakterize bir bozukluktur. Fizik ya da psikolojik belirti ve bulguların taklidi biçiminde ortaya çıkabilir. Psikolojik belirtilerle giden ve psikiyatrik bozuklukları taklit eden yapay bozukluk daha ender görülmekte, buna karşılık yol açtığı tanı, tedavi ve izleme sorunları açısından özellikle dikkati çekmektedir. Bu yazıda, yirmi dört yıldır yakınmaları olan, şizofreni ve temporal lob epilepsisi dahil olmak üzere çok sayıda tanı konmuş, değişik kurumlarda ayaktan ve uzun süreli yatışlarla izlendikten sonra, kliniğimize özkıyım düşünceleri nedeniyle yatırılan ve daha sonra uzun süreli ayaktan izleme sonucunda yapay bozukluk tanısı konan kırk sekiz yaşında bir kadın hasta sunulmakta, bu bağlamda yapay bozukluğun tanı ve tedavisinde dikkat edilmesi gereken noktalar tartışılmaktadır.

A case report of factitious disorder that mimics multiple mental disorders

Factitious disorder is charecterized by intentionally production of illness signs and symptoms to assume sick role. It can present with both physical and psychological signs and symptoms. Factitious disorders with psychological symptoms that mimic psychiatric disorders are rare but take considerable interest because of the problems they cause in diagnosis, treatment and follow-up. In this paper we present^ a forty-eight years-old woman who had symptoms for twentyfour years and had been diagnosed with various illnesses including schizophrenia and temporale lobe epilepsy. After being followed-up in various outpatient and inpatient clinics with long hospital stays, she had been hospitalized in our clinic for suicidal ideations and had been diagnosed as factitious disorder after a long follow-up. In this paper, in the light of the patient's signs and symptoms, important points in diagnosis and treatment of factitious disorders are discussed.
