Konstipasyon saptanan olgularımızın değerlendirilmesi

Giriş ve amaç: Konstipasyon toplumda oldukça sık görülen, sıklığı kullanılan tanımlara göre farklılık göstermekle birlikte %2-28 arasında değişen bir gastrointestinal problemdir. Konstipasyonun tanımlanmasında tam bir objektivitenin olmaması sonucunda bu problemin sıklığı, patogenezi, tedavisi konusunda tartışmalı sonuçlar ortaya çıkmaktadır. Yapmış olduğumuz bu çalışmada hastanemiz Gastroenteroloji polikliniğine değişik nedenlerle başvurmuş hastalarda konstipasyon ve özelliklerini belirlenmeyi amaçladık. Gereç ve yöntem: Çalışmamızda 1 Ocak-30 Haziran 2005 tarihleri arasında hastanemiz Gastroenteroloji polikliniğine değişik nedenlerle başvurmuş hastaların dosyaları konstipasyon ve özellikleri açısından retrospektif olarak incelendi. Konstipasyon tanısı için Roma II tanı kriterleri kullanıldı. Bulgular: Çalışmaya alınan 1000 hastadan 200'ünde (%20) konstipasyon saptandı. Konstipasyon saptanan hastaların 146'sı (%73) kadın, 54'ü (%27) erkekti. Yaş ortalamaları 53.45±16.72 yıl idi. Hastaların 48'inde (%24) konstipasyon primer başvuru yakınması iken, 152'si (%76) başka nedenlerle başvurmuştu. Altmış üç hastada (%31.5) haftada 3 veya daha seyrek dışkılama baskın şikayet iken, 76 hastada (%38) zorlu dışkılama, 61 hastada (%30.5) sert dışkılama ana yakınma idi. Hastaların 34'üne (%17) İBS tanısı konmuştu. Yüz altmış hastada (%80) eşlik eden bir hastalık bulunmazken, 24'ünde (%12) DM, 12'sinde (%6) hipotiroidizm, 3'ünde (%1.5) hipopotasemi ve 1'inde (%0.5) üremi vardı. Hastaların 27'si (%13.5) laksatif kullanmaktaydı. Sonuç: Çalışmamızda hastanemiz Gastroenteroloji polikliniğine başvuran hastalarda konstipasyon sıklığını %20 olarak bulduk. Konstipasyonun hayat kalitesini etkilemesi, tetkikler ve laksatif kullanımı nedeniyle maliyetleri arttırması ve önemli hastalıklar ile birarada bulunabilmesi nedeniyle dikkatli olunması gereken bir problem olduğunu düşünüyoruz.

Evaluation of patients with constipation

Background/aim: Constipation is a frequent gastrointestinal problem. Prevalence of constipation varies between 2-28% according to different definitions. Because the definition of constipation is not definitive, it is difficult to estimate the prevalence, pathogenesis and treatment of this problem. In this study we aimed to determine constipation and its features in outpatients admitted to our Gastroenterology polyclinic for different causes. Materials and methods: In our study the files of patients admitted to the Gastroenterology polyclinic between 1 January-30 June 2005 were evaluated retrospectively for constipation and features. Rome II criteria were used for diagnosis of constipation. Results: A total of 200 patients with constipation (146 females, 54 males) were enrolled into the study. Mean age was 53±16.72 years. Constipation was the primary complaint of 48 (24%) patients and the remaining 152 (76%) were admitted with other complaints. Less than three bowel actions per week was the main symptom in 63 (31.5%) subjects, straining in 76 (38%) and hard stools in 61 (30.5%). Thirty-four (17%) patients were diagnosed as irritable bowel syndrome (IBS). There was no accompanying disease in 160 (80%) patients, but 24 (12%) had DM, 12 (6%) had hypothyroidism, 3 (1.5%) had hypopotassemia and 1 (0.5%) had uremia. Twenty-seven (13.5%) patients were using laxatives. Conclusion: In this study the prevalence of constipation in outpatients admitted to our Gastroenterology polyclinic was 20%. We think that patients with constipation have to be evaluated more carefully because of its disturbance of life quality, increasing costs, laxative use and accompaniment with important diseases.
