Yüz maskesi uygulamasının uzun dönem sonuçlarının değerlendirilmesi

AMAÇ: Çalışmanın amacı, yüz maskesi uygulamasının uzun dönem etkilerini, büyüme ve gelişim potansiyeli az kalmış ya da hiç kalmamış olan bireylerde değerlendirmektir.GEREÇ VE YÖNTEM: Bu araştırmanın materyalleri, üst çene yetersizliği ya da üst çene yetersizliği ve alt çene ileriliği kaynaklı iskeletsel Sınıf III malokluzyon ile birlikte ön çapraz kapanış veya baş başa keser ilişkisi ve/veya Sınıf III büyük azı ilişkisine sahip, Delaire tipi yüz maskesi ile tedavi edilmiş 17 bireyin (9 kız, 8 erkek; ortalama yaş 11.1 yıl) uygulama öncesi (T1), uygulama sonrası (T2) ve uzun dönem takip sonu (T3) lateral sefalometrik ve el-bilek filmlerinden oluşturuldu. T3 döneminde, büyüme ve gelişim potansiyeli azalmış ya da hiç kalmamış bireyler çalışmaya alındı. Ortalama takip süresi (T3-T2) 4.7 yıldı.BULGULAR: SNA açısının ve A┴Vert T mesafesinin uygulama döneminde (T2-T1) ve toplam sürede (T3-T1) önemli düzeyde arttığı bulundu (sırasıyla, p<0.001, p<0.01). SNB açısı ve B┴Vert T mesafesinin sadece takip döneminde (T3-T2) önemli düzeyde arttığı bulundu (sırasıyla p<0.001, p<0.01). ANB açısının uygulama döneminde ve toplam sürede önemli düzeyde arttığı (sırasıyla, p<0.001 ve p<0.01) takip döneminde önemli düzeyde azaldığı belirlendi (p<0.01).SONUÇ: Yüz maskesi uygulamasından sonraki takip döneminde bireye özgün büyüme ve gelişim yönünün tekrar ortaya çıkması sonucunda nüks görülebilmektedir. Bu da Sınıf III malokluzyona sahip bireylerin tedaviden sonra takibinin gerekliliğini ortaya çıkarmaktadır.

Evaluation of the long-term results of facemask

OBJECTIVE: The aim of the present study was to evaluate the long-term results of face mask when growth potential of the patients were close to cease or ceased.MATERIALS AND METHOD: The material consisted of pre-treatment (T1), post treatment (T2), and long-term (T3) lateral cephalograms and hand-wrist radiographs of 17 patients (9 female, 8 male; mean age: 11.1 years) with skeletal Class III malocclusion characterized by maxillary deficiency or maxillary deficiency and mandibular prognatism with anterior crossbite or tet-a-tet incisor and/or Class III molar relationship and treated with Delaire type face mask. Subjects whose growth potentials were close to cease or ceased at the T3 period were selected for the study. The mean follow-up period (T3-T2) was 4.7 years.RESULTS: Significant increases in SNA angle and A┴VertT distance were observed during face mask therapy (T2-T1) and at overall period (T3-T1; p<0.001, p<0.01, respectively). SNB angle and B┴Vert T distance increased only during the follow-up period (T3-T2; p<0.001, p<0.01, respectively). For the ANB angle, significant increases were found during face mask therapy and at overall period (p<0.001, p<0.01, respectively); however, a significant decrease was found during the follow-up period (p<0.01).CONCLUSION: After face mask therapy, relapse may occur during follow-up due to re-occurrence of the original growth pattern. These findings suggest the necessity of follow-up after treatment in patients with Yassıla III malocclusionduring growth and development.
