Psikiyatrik Bakımda Orem’in “Öz Bakım Eksikliği Kuramı”nın Kullanımı: Olgu Sunumu

Hemşirelik uygulamalarında hemşirelik kuram ve modellerinin kullanımı, uygulamaların kavramsal bir temele dayandırılmasını, hemşirelere bağımsız olabilme ve kendi bakış açılarıyla uygulama yapma olanağı sağlar. Orem’in Öz Bakım Eksikliği Kuramı birçok hasta grubunda sık kullanılan bir modeldir. Bu makalede bir üniversite hastanesinin psikiyatri kliniğinde Obsesif Kompulsif bozukluk tanısıyla yatarak tedavi gören bir hastayı ele alan bu olgu sunumunda, Orem’in Öz Bakım Eksikliği Kuramı temelinde bireyin öz bakımını sağlamak, bireyin öz bakım yeterliğini artırmada bireye destek sağlamak ve bakımın sürdürülebilirliği için bireyi ailesi ile birlikte ele alan destekleyici eğitici hemşirelik bakım süreci sunulmuştur. Hastaya ait veriler; kuram doğrultusunda temel durumsal faktörler, evrensel, gelişimsel ve sağlıktan sapmada öz bakım gereksinimleri olmak üzere dört alana yönelik toplanmıştır. Hemşirelik bakımı, “hemşirelik süreci sistemi” ne göre; verilerin değerlendirilmesi, hemşirelik tanısı, amaçlar, müdahaleler hemşirelik sistemi ve yardım metodu ve değerlendirme alanları çerçevesinde sürdürülmüştür. Değerlendirme, hastanın taburcu olduğu beşinci günün sonunda yapılmıştır. Kuram, hastanın ve sorunların çok yönlü değerlendirilmesinde hemşirelik uygulamalarına kolaylık sağlamıştır

Application of Orem"s Self-Care Deficit Theory To Psychiatric Care: a Case Report

The use of nursing theories and models in nursing practices enables them to be based on a conceptual basis, to be independent of nurses and to practice with their own perspective. Orem’s Self-Care Deficit Theory is a model commonly used in many patients. In this article, a nursing care process conducted according to Orem’s Self-Care Deficit Theory is presented for a patient who is receiving inpatient treatment with Obsessive-Compulsive disorder in a psychiatric clinic of a university hospital. On the basis of theory, a supportive educational nursing care process that provides self-care for the individual who provides support to other individuals to increase self-care competence and for the sustainability of care and handles the individual with his/her family, is presented. Patient’s data were collected for four areas according to the theory: basic conditioning factors, universal, developmental and health deviation self-care requisites. Nursing care, according to the “nursing process system” has been carried out within the framework of evaluation of the data, nursing diagnosis, goals, interventions nursing system and method of helping and evaluation areas. The assessment was made at the end of the fifth day that the patient was discharged. The theory facilitated the nursing practice in the multidimensional assessment of the patient and the problems
