Apikal rezeksiyon operasyonlarında trombositten zengin fibrin uygulaması: Olgu serisi
Apikal rezeksiyon sonrası oluşturulan kemik kavitelerinin iyileşmeleri sırasında her zaman istenilen sonuçlar elde edilememektedir. Bu nedenle apikal rezeksiyon kavitelerinde rejeneratif tedaviler uygulanmaya başlanmıştır. Trombositten zengin fibrin (TZF) bireyin kanının santrifüj edilerek trombosit ve büyüme faktörlerinden zengin bir membran elde etmeyi sağlayan ikinci nesil trombosit ürünüdür. Bu vaka serisinde apikal rezeksiyon işlemi sonrası operasyon bölgesine yerleştirilen TZF’nin klinik ve radyografik olarak sonuçları sunulmuştur. Endodonti kliniğinden apikal rezeksiyon endikasyonu ile sevk edilen 3 hastaya cerrahi işlem sırasında periapikaldeki tüm nekrotik ve iltihaplı dokunun küretajı yapılmış ve defekt bölgesine hastalardan alınan venöz kanın santrifüj edilmesiyle hazırlanan TZF uygulanarak yara yeri süture edilmiştir. Hastaların periyodik kontrollerindeki klinik ve radyografik sonuçlarına göre; TZF’nin kemik kavitesi boşluğunun rejenerasyonunda etkili olduğu ve konvansiyonel cerrahi tedaviye alternatif bir rejeneratif tedavi şekli olabileceği kanısındayız.
Platelet rich fibrin application in apical resection operations: Case series
During the healing of bone cavities formed after apical resection, the desired results are not always obtained. For this reason, regenerative treatments have begun to be applied in apical resection cavities. Platelet-rich fibrin (PRF) is a second-generation platelet product derived from the blood of the same individual and used to obtain a membrane rich in plateletes and growth factors. In this case series, clinical and radiographic results of PRF placement onto the operation area after apical resection were presented. All the necrotic and inflamed tissue were removed from the periapical region during the surgical procedure of three patients referred to the endodontic clinic with indication of apical resection. Following application of PRF to the defect area which is prepared by centrifugation of the venous blood taken from the patients, wound site was sutured. According to the clinical and radiographic results of the patients at periodic controls, PRF appeared to be effective in the regeneration of bone gap space providing an alternative to conventional surgical treatment.
