Periton Diyalizi Hemşirelerinin Periton Diyalizi Uygulanan Hastalara Verdiği Telefon Desteğinin Değerlendirilmesi

Amaç: Periton diyaliz (PD) hemşireliği, son dönem böbrek yetmezliği olan hasta-ların diyaliz tedavi modalitesi seçimine yardımcı olacak eğitim ile başlamakta,kateterin takılması, diyalizin uygulanması, gelişebilecek komplikasyonlar ile ilgilieğitimler ile devam etmektedir. Hastaların diyaliz tedavisi sürecinde karşılaştığıproblemlerin zaman zaman acil müdahale gerektirmesi yanında hastalarınkendilerini güvende hissetmek istemeleri ve sağlık ekibinin yanlarında olduğunuhissetmek istemeleri telefon ile destek verilmesini gerekli kılmaktadır. Buçalışmanın amacı, PD hastalarının evde tedavilerini sürdürürken PD hemşiresinintelefon ile desteğine hangi sıklıkta ve hangi nedenlerle ihtiyaç duyduğununbelirlenmesidir.Gereç ve Yöntemler: Merkezimizde takipli PD hastalarının Ekim 2018-Şubat2019 tarihleri arasında merkezimizin iki periton diyaliz hemşiresine yaptığıtelefon aramaları, arama nedeni ve arama süresi ile kaydedildi.Hastalarındemografik verileri ve periton diyalizi özellikleri kaydedilerek yapılan aramalarlailişkisi araştırıldı.Sonuç: Yaş ortalaması 49,6±13,6 olan %53,1’i kadın (n=26), %46,9’u erkek(n=23), 49 hasta çalışmaya alındı. Hastaların %53,1’i aletli PD (n=26), %46,9’usürekli ayaktan PD (n=23) uygulamaktaydı. Toplamda 49 hasta 649 kez aramayaptı. Aramaların toplam süresi 598 dakika, bunun 361 dakikası mesai içi, 237dakikası ise mesai dışı aramaydı. 46 hasta tıbbi nedenle ortanca 5 defa (min-maks:1-21), 22 hasta sosyal sebeple ortanca 3 defa (min-maks: 1-6), 29 hasta tekniksebeple ortanca 5 defa (min-maks: 1-16) aradığı görüldü. Aramalar karşılaştırıldı-ğında, sosyal ve tıbbi nedenle mesai içi ve mesai dışı arama süre ve sayılarıarasında fark bulunmazken; teknik sebeple yapılan aramaların süresinin mesaidışında anlamlı olarak fazla olduğu görüldü (p=0,009). Çalışmamız, özellikleteknik sorunların mesai saati dışında da destek gerektirdiğini göstermiştir. Başarılıbir PD süreci için gerekli destek özellikle PD hemşireleri tarafından hem sağlıkmerkezinde hem de evde devam ettirilmektedir.

Assessment of Telemedicine Support of Peritoneal Dialysis Nurses to Peritoneal Dialysis Patients

Objective: Peritoneal dialysis (PD) nursing begins with education that will assistin the selection of dialysis treatment modality in patients with end-stage renalfailure, and continues with education in catheter insertion, PD performing,complications that may arise in PD in daily life. Most of the time, telephonesupport is a necessity because of the problems requiring urgent intervention facedby the patients. Additionally, patients need to feel that they are not alone whiletheir PD treatment process. The aim of this study is to determine how often andfor what reason PD patients are needed for the support of the PD nurses viatelephone in our center.Methods: The telephone calls made by PD patients to our PD nurses in our centerbetween October 2018 and February 2019 were recorded with the reason andduration of the calls. The demographic data and clinic characteristics of thepatients were recorded.Results:Of the patients 53,1% were female (n = 26) and 46,9% were male (n =23). The mean age of the patients was 49.6 ± 13,6. 53,1% of the patients wasperforming automated PD (n = 26) and 46.9% of them was performing continuousambulatory PD (n = 23). A total of 49 patients performed 649 calls. The totalduration of the calls was 598 minutes, of which 361 minutes were during workinghours and 237 minutes were after working hours. 46 patients made calls withmedian 5 times (min-max: 1-21) for medical reasons; 22 patients made calls withmedian 3 times (min-max: 1-6) for social reasons, 29 patients made calls withmedian 5 times for technical reasons (min-max: 1-16). Additionally, 32 patientsmade calls with median 3 times (min-max: 1-21) for the prescription procedures.The duration of the calls made for technical reasons was found to be significantlyhigher outside the working time (p = 0.009). Our study showed that especiallytechnical problems require support outside the working hours. Support for asuccessful PD process is provided for, by PD nurses both at the health center andat home.
