VZA/AHP BÜTÜNLEŞİK YÖNTEMİ İLE PERFORMANS ÖLÇÜMÜ: ANKARA’DAKİ KAMU HASTANELERİ ÜZERİNE BİR UYGULAMA

Bu çalışmanın amacı kamu hastanelerinin göreceli etkinliklerinin ölçülmesi ve etkin olan/olmayan hastanelerin belirlenmesidir. Çalışma kapsamında Ankara’da faaliyet gösteren 26 kamu hastanesi değerlendirilmiştir. Hastanelerin sağlık hizmeti üretmek için kullandığı 4 girdi ile ürettiği 5 çıktı belirlenmiş ve bu girdi ve çıktılarla iki farklı VZA modeli oluşturulmuştur. Bunlardan ilki, girdi ve çıktılarında ağırlık kısıtlaması olmayan VZA modeli, ikincisi ise girdi ve çıktılarında AHP yardımıyla elde edilen ağırlıkların kullanıldığı VZA modelidir. Her iki model de ayrı ayrı çözülerek etkinlik ölçümü gerçekleştirilmiştir. İlk modelin çözülmesi sonucunda 13, ikinci modelin çözülmesi sonucunda ise 10 hastane etkin bulunmuştur. Diğer bir ifadeyle ilk VZA modelinde etkin bulunan 3 hastanenin ikinci modelde etkin olmadığı görülmüştür. Ayrıca ortalama etkinlik skoru ilk durumda % 91, ikinci durumda ise % 86 olmuştur. Etkin olan ve etkin olmayan hastaneler bu etkinlik skorlarını stratejik kararlarında kullanabileceklerdir. Etkin hastanelerin konumlarını korumaları, etkin olmayanların ise etkin olabilmek için referans kümelerinde yer alan etkin birimleri örnek almaları beklenmektedir. Bu çalışmadan elde edilen sonuçların, ülkenin sağlık politikasını belirleyen karar vericiler ile hastane idarecileri ve çalışanları tarafından yol gösterici olarak kullanılabileceği umulmaktadır. Son olarak çalışmanın kısıtlarından bahsedilmiş ve gelecekte bu konuda araştırma yapacaklar için bazı öneriler geliştirilmiştir.

PERFORMANCE ASSESSMENT USING DEA/AHP INTEGRATED METHOD: AN APPLICATION ON PUBLIC HOSPITALS IN ANKARA

The aim of this study is to measure the relative efficiency of the public hospitals and to determine the efficient/inefficient hospitals. 26 public hospitals in Ankara were included in this study. 4 inputs the hospitals use and 5 outputs they generate were specified while providing health service and two different DEA models were established using these same inputs and outputs. The first DEA model uses the inputs and outputs without weights and the second DEA model uses the inputs outputs with the weights derived from AHP. Each models were run separately and efficiency measurement was made. As a result of running the first DEA model, 13 hospitals were found to be efficient and this was 10 for the second model. In other words, it was seen that 3 hospitals that were efficient in the first DEA model became inefficient in the second model. In addition, the average efficiency score was 91% in the first state and 86% in the second state. Efficient and inefficient hospitals could use these efficiency scores in their strategic decisions. It is expected that efficient hospitals will maintain their position and inefficent hospitals will take the efficient hospitals in their reference sets as an example to become efficient. It is hoped that the results of this study can be used as leading principles by decision makers of health policy, and by hospital managers and staff. Finally, constraints of this study and the research areas are pointed out for future research on this subject.
