KEMOTERAPİ TEDAVİSİ ALAN HASTALARDA PROGRESİF GEVŞEME EGZERSİZLERİ VE YÖNLENDİRİLMİŞ İMGELEM UYGULAMASININ KEMOTERAPİ SEMPTOMLARI VE YAŞAM KALİTESİ ÜZERİNE ETKİSİ

ÖZAmaç: Çalışmanın amacı, kemoterapi tedavisi alan kanser hastalarında progresif gevşeme egzersizleri ve yönlendirilmiş imgelem uygulamasının kemoterapi semptomları ve yaşam kalitesine etkisini incelemektir.Yöntem: Çalışma, Fırat Üniversitesi Tıp Fakültesi Hastanesi Onkoloji BD kemoterapi ünitesinde ayaktan ilk kez kemoterapi tedavisi alan hastalar üzerinde 35 deney ve 35 kontrol olmak üzere toplam 70 hasta ile yürütülmüştür. Veriler; Hasta bilgi formu, Kemoterapi Semptom Değerlendirme Ölçeği, bulantı-kusma kayıt çizelgesi ve EORTC QLQ-C30 Yaşam Kalitesi Ölçeği ile toplanmıştır. Verilerin analizinde Independent Samples t testi, Paired-Samples t Testi, Mann Whitney U Testi ve Ki-kare testleri kullanılmıştır.Bulgular: Progresif gevşeme egzersizleri ve yönlendirilmiş imgelem uygulaması sonrasında semptomların sıklığı karşılaştırıldığında tedaviden sonra bulantı ve kusma, halsizlik hissetme, uyuma güçlüğü ve endişeli ve sıkıntılı hissetme semptomlarının deney grubunda istatistiksel olarak daha az yaşandığı belirlendi (p<0.05). Deney ve kontrol gruplarının yaşam kaliteleri karşılaştırıldığında son-test deney ve kontrol grubu hastaların genel iyilik durumu, fiziksel ve emosyonel durum, semptomlardan ise yorgunluk, bulantı kusma ve ağrı yönünden gruplar arasında istatistiksel olarak anlamlı farklılık olduğu tespit edildi (p<0.001).Sonuç: Sonuç olarak yönlendirilmiş imgelem ve gevşeme egzersizleri uygulamalarının kemoterapi tedavisinin bulantı, kusma, ağrı, iştah, uyku problemleri, halsizlik, yorgunluk, endişe ve üzüntü hissetme, karamsar ve sıkıntılı hissetme gibi semptomlar üzerinde olumlu etkisinin olduğu ve hastalarda yaşam kalitesini arttırdığı belirlenmiştir.

Influence of Progressive Relaxation Exercises and Guided Imagination Applications on the Chemotherapeutical Symptoms and Quality of Life of Patients Undergoing Chemotherapy

ABSTRACTAim: The research was conducted as a pre-test and post-test control group quasi-experimental model to evaluate the influence of Progressive Muscle Relaxation Exercises (PMR) and Guided Imagination  (GI) application on the chemotherapeutical symptoms and quality of life of those patients under chemotherapy treatment.Methods: The population of this research is those patients who admitted the chemotherapeutical treatment unit at University of Fırat Faculty of Medicine Oncology Department the first time. The research was conducted between March and October 2010 on 70 who were selected randomly from the population. Data were collected by a questionnaire, Chemotherapy Symptom Evaluation Scale, Nausea-Vomit Record Table and EORTC QLQ-C30 Quality of Life Scale. Statistical analysis of data were analyzed by Independent Samples t test and Paired-Samples t Test, Mann Whitney U Test, chi-square tests.Results: In terms of frequency of symptoms after the progressive relaxation exercises and guided imagination application, symptoms such as nausea and vomiting, feeling weak, sleep disorder and feeling anxious and distressful after the treatment were determined to be experienced statistically less frequently in the experimental group (p<0.05). In comparison of pre-post test score means of life quality, the difference in general well-being condition, physical function, emotional condition, and tiredness and pain symptoms in the experimental group was statistically significant (p<0.001). Conclusion: These result indicate that PMR and GI had positive effects on nausea and vomiting, feeling weak, sleep disorder, anxious, distressfull and in the quality of life of patients undergoing chemotherapy.
